Q: What to bring to get a JSA SLCard?
A:
Students: Photo ID (in addition, your status will be confirmed with the University Registrar)
Faculty: Photo ID (in addition, your status will be confirmed with the your appointment letter)
Staff: Photo ID (in addition, your status will be confirmed with Human Resources)
Students, Faculty, and Staff will receive their first JSA SLCard free of charge. Replacement JSA SLCards will cost $15.00 payable by cash, check, or with JSA Money debit account funds.
Alumni: Photo ID and $10.00 (in addition, your status will be confirmed with Alumni records)
Alumni, Spouses, and Dependents will be charged $10.00 for their first JSA SLCard. Replacement JSA SLCards will cost $10.00 payable in cash, check, or with JSA Money debit account funds.

Q: Lost and/or stolen JSA SLCards?
A:
If you have lost your JSA SLCard or your JSA SLCard has been stolen, report it immediately! You may do this anytime by clicking on the Lost/Stolen Card link at the right.
You may also contact the JSA SLCard Office during business hours to report your JSA SLCard lost.
You can report a lost JSA SLCard at any attended retail location that accepts the JSA SLCard (except the Bookstore).
"Lost" JSA SLCards are reactivated in the JSA SLCard Office only. For your protection, all JSA SLCards used by someone other than the cardholder will be confiscated.
Unauthorized expenditures up to $500 will be the responsibility of the cardholder for lost JSA SLCards not reported within 48 hours.
